Immunization

Immunizations are an important part of good health care for children.  If you are unsure or have any concerns about vaccinations, Dr. Daill will be available for discussion on this topic. 

Your child’s immunizations will begin in the office at 2 months of age and follow our published immunization schedule. You will be asked to sign a consent form each time your child receives a vaccine in this office. The consent signifies your understanding of vaccines and possible side effects. We use combination vaccines when possible to minimize the number of injections to your child.

If you choose not to immunize your child after discussing it with Dr. Daill, a release will be required to receive other medical treatment at Dundee Pediatrics.


After Hours Contact

For patients already under the care of Dundee Pediatrics, 
 Dr. Daill is available to consult after hours when necessary.  Phone numbers and instructions for this service are available during normal business hours to current patients of Dundee Pediatrics.  Prior to calling, please see tips on preparing to call the Doctor.

For those who are not yet patients of Dundee Pediatrics, please contact us for an appointment during normal business hours. 

In case of an emergency, call 911
or proceed to an emergency room or urgent care clinic.

Payment for Services

Copayments:     Copayments required by your insurer will be due at the time of service. 

Non-Covered Services:      
Dundee Pediatrics PLLC renders medical care in accordance with the guidelines of the American Academy of Pediatrics.  If your plan does not cover services rendered, payment will be the responsibility of the patient.

Deducibles and Co-Insurance
:      Patients / Guardians will be responsible for prompt payment of deductibles and Co-Insurance upon presentment with a statement. 

Uninsured Patients:    Services rendered to patients without health care coverage will be due at the time of service.

Acceptable Methods of Payment:  Dundee Pediatrics, PLLC accepts Cash, Personal Checks, Visa, Mastercard, Visa Debit Cards and Mastercard Debit Cards.

Non-Payment for Services:  Services shall not be rendered to patients with accounts greater than 60 days delinquent.  Accounts more than 60 days delinquent shall be turned over to collections.
